Infant Botulism

Wound Botulism
Wound botulism occurs when the bacteria enter a wound and produce toxin. It may develop in deep or poorly treated wounds and has also been associated with injecting certain drugs.
Adult Intestinal Botulism
This rare form occurs when spores grow and produce toxin inside an adult’s intestines. It is more likely in people who have particular intestinal conditions or who have recently undergone certain gastrointestinal procedures.
Medical Botulism
Botulinum toxin is used in controlled doses for medical and cosmetic treatments. In rare cases, excessive or improperly administered toxin may spread beyond the injection area and cause symptoms similar to botulism.
Symptoms of Botulism
Botulism usually causes weakness that begins in the face and upper body and then moves downward. A person may remain fully conscious while losing control of certain muscles.
Common symptoms include:
- Blurred or double vision
- Drooping eyelids
- Difficulty speaking
- Difficulty swallowing
- Dry mouth
- Facial weakness
- Muscle weakness
- Shortness of breath
- Paralysis
Foodborne botulism may also cause nausea, vomiting, stomach pain, diarrhea, or constipation.
Infants may show different symptoms, including constipation, weak crying, poor feeding, reduced facial expression, unusual tiredness, difficulty holding up the head, and floppy or weak muscles.

Is Botulism Contagious?
Botulism is not normally spread from one person to another. People usually develop the illness after consuming contaminated food, after spores grow inside the intestines, or when bacteria enter a wound.

How Is Botulism Diagnosed?
Doctors diagnose botulism by examining the patient, reviewing symptoms, and asking about possible exposure to contaminated food, wounds, or botulinum toxin treatments. Laboratory tests may be used to detect the toxin or the bacteria in blood, stool, food, or wound samples.
Because laboratory confirmation may take time, doctors may begin treatment based on symptoms and suspected exposure.
Treatment
The main treatment for botulism is an antitoxin. The antitoxin prevents toxin that is still circulating in the bloodstream from causing further nerve damage. However, it cannot immediately reverse paralysis that has already occurred. For this reason, treatment should begin as early as possible.
People with breathing difficulties may require mechanical ventilation. They may also need nutritional support, physical therapy, and rehabilitation while damaged nerves recover. Recovery can take weeks or months.
Infant botulism is treated with a specialized immune globulin. Wound botulism may also require antibiotics, wound cleaning, or surgery to remove infected tissue.
Preventing Botulism
Botulism can be prevented by following safe food preparation and preservation practices. Home canning must be performed using approved temperatures, pressures, and processing times. Food from leaking, swollen, cracked, or damaged containers should be discarded without tasting it.
Additional preventive measures include:
- Never giving honey to a child under one year old
- Refrigerating oils containing garlic or herbs
- Keeping baked potatoes wrapped in foil hot until served or refrigerating them promptly
- Following reliable instructions when fermenting, preserving, or canning food

- Avoiding the use of injected illegal drugs
- Receiving botulinum toxin treatments only from qualified healthcare professionals
